hCG measurements in pregnancies following in vitro fertilization (IVF)

A pregnancy test, in an IVF cycle, is generally scheduled approximately two weeks after an embryo transfer.  This involves a blood test to measure the hormone of pregnancy – hCG (human chorionic gonadotropin).  If positive (greater then 5 mIU/ml) the test is repeated two days later and it should rise appropriately.  Initial studies had suggested that in naturally conceived pregnancies, the 2-day rise should be at least 66%.   A slow rise was believed to be associated with an abnormal intrauterine pregnancy or a tubal pregnancy.  More recent studies have shown that the rise could be as slow as 53% in 2 days and the pregnancy could still turn out to be normal.

What about IVF pregnancies?  Do the same rules apply or should we use different criteria for these pregnancies?  Dr. Beata E. Seeber (from Innsbruck Medical University, Innsbruck, Austria) discussed this very issue in a recent paper in the Journal “Fertility and Sterility”.  The following are some of the important points presented in the paper.

  • hCG levels determined 13-15 days after fertilization are higher in pregnancies from cleavage-stage day 3 embryos than those achieved through the transfer of blastocysts on day 5.  Nonetheless, the percentage increase in serial measurement of HCG did not differ between these groups.
  • A single hCG level cannot be used to differentiate between miscarriages and ectopic pregnancies.
  • A combination of hCG levels on day 15 and 22 after embryo transfer can predict pregnancy outcome.   When the hCG level on day 15 was >150 mIU/mL and an hCG day 22/hCG day 15 ratio of > 15 with a specificity of 94% and sensitivity of 47% for normal pregnancy. Conversely, when the hCG on day 15 was below 150 mIU/mL and the ratio of hCG on day 22/hCG on day 15 was below 15, there was an 84% chance of an abnormal pregnancy.
  • There is no single hCG level that defines a normal pregnancy.  The rate of increase in hCG levels, however, is important.  The higher the percentage of rise in hCG drawn 2 days following the initial value, the better the live deliver rate.
  • When compared to spontaneous pregnancies, the average doubling time of hCG level in IVF pregnancies appears to be the same (about 1.5 days.).
  • In patients with ovarian hyperstimulation syndrome (OHSS), hCG levels should be interpreted carefully.  These patients often have fluid that is shifting from the blood vessels to the abdominal cavity making the blood concentrated.  When IV fluids are given, the blood levels can get diluted leading to variations in hCG levels.  One can often see hCG levels that are flat or not rising normally in these patients only to show a normal rise when the OHSS is stabilized.
  • What about twin and triplet pregnancies?  These are more common with IVF and therefore deserve special discussion.  The initial hCG levels are higher in multiple pregnancies than for singletons.  But, the rate of rise is similar for both groups.

hCG levels are often used in combination with ultrasound to confirm the diagnosis of a normal vs. abnormal pregnancy.  In very early pregnancy, the hCG levels are the primary diagnostic tool.  In later pregnancy, ultrasound and clinical examination are often used in combination with hCG levels.

    Our first IVF cycle was two years ago. We did it in Seattle so we were away from home. But it was the week of the summer Olympics so we got to watch all the evtnes as we progressed through our egg retrieval and embryo transfer. Talk about a nice distraction. I also rely on Friends when I need a fun distraction its light and funny, and keeps my mind off IF stuff. When we do our next IVF cycle (hopefully in the next 6 months), we plan to be in Seattle again, but we will have our son w/ us, so he will keep us occupied enough.Its the stupid in between IVF cycles that drives me crazy. I have taken up a lot of craft projects, which I find very therapeutic. But the driving force getting me through the waiting time right now is our home improvement projects. There are quite a few that absolutely have to get done before another baby comes along, so my goal is to complete them before our cycle even gets underway. Not only does it occupy my waiting time, but its rewarding to fix up my house. Oh, and I am also doing a half-marathon before our next cycle. Its on my bucket list.

    i had two embryo transfer on 4th april 2013 with a donor egg(7 cells-2(a grade))on 1st april the ER was done.Dr suggested for Beta HCG on 18th april(14th day after ET)today the 6th day what are the symptoms should have for pregnancy.What shoul be the HCG level on 14th day.Also suggest the precautions & what else to be done to make the thing positive,as i m already taking progesteron(evadiol) 8mg ,susten(400) vaginal twice & ATP once a day & also clinone gel .5 vaginal gel once at night.

    • You may have symptoms from the medications you are taking but it is too early to have pregnancy symptoms. There is no one good number for a positive pregnancy test. The hCG level 14 days after embryo transfer is generally greater than 100 mIU/mL. It can be higher in cases with multiple pregnancy. I have had several patients with lower levels that have gone on to have healthy babies. Good luck!

      My wife and I just had FET on April 26. Her symptoms between then and day 14 were almost that of her period. She experienced sore breasts and some bleeding. She was certain it was bad news. We were pregnant. Dont streas at all! have faith, wait it out, and dont touch a hpt.
      Day14 beta hcg was 814. Day18 beta hcg was 6,100.
      We go back on may 28 for next tests. Fingers crossed!!

    I just got a call from my nurse and its positive. My Husband and I are finally going to be parents after 2 years of trying. Four failures of IUI.

    This is my first IVF cycle (2 embryos transfer) and I was nervous. After finding out that we are expecting, I was excited. Although, my nurse mention that my HcG is a bit low (62) and I should be above 75. Should I be worry? How can I know that both embryos attached?

    • Enjoy the moment! You now on the other side of the fence! We have had literally hundreds of patients with the first hCG level similar to yours and have done well. Good luck!

    I had gone through ivf icsi with 3 fresh et. After 14 days my hcg level was 7.5. My doc said neg. She asked me to stop. Medicine as I heard aft 5 hcg level it can b positive. I refused to stop medicine. I hv decided to retest aft 3 days. Is there any chances to get positive result?

    • Hi Neha

      Your first hCG level (14 days after embryo transfer) is low. It would be extremely unusual for a pregnancy to progress normally with such a low first level. However, it should be followed to < 5 mIU/mL (negative). We have seen tubal pregnancies and miscarriages with this scenario. You are correct to retest after 3 days.

      I’m sorry this is the outcome with your third cycle. The “pathway to pregnancy” (our logo at InVia) can be tough, but do persist and you will attain your goal.

        Hi doc, thanks for ur reply. Today I retest n again it was neg. My report was good n I responded well even embryo quality was gi. I was very hopefull.I m very upset. I don know wht went wrong.Doc asked me to take one month rest thn cm back. I still hv 8 frozen embryos. Kindly guide me accordingly. I m 30 yrs old n tht was my first ivf. Do I need to continue any medicine meanwhile? Wht kind of precautions should I take?

    my sister is undergoing IVF, this is her fourth cycle, there were 3 embryos transferred, two of which are of first digree. yesterday “after 12 day” she had an hCG test and it was 3, the doctor asked her to come back next saturday to re-test, and he said that there is a chance that it will be elevated. yet she is very worried .. if yoy could tell me if there really a chance that it wil be elvated and if the embryos could perfectly implant !!

    This is my 3rd ivf…today my beta hcg level is 4.5….it will increase after 2 days….
    They transfer 2 embryos 1 is gud n 1 is moderate.l..there is chance to get pregnant….b’cos i have some pregnancy symptoms….please reply me…

    • 4.5 is not a good hCG value. I am presuming it is two weeks since your embryo transfer. In our lab this value is interpreted as a negative pregnancy test.
      Sorry, I do not have good news for you based on the information you have provided.
      The symptoms could be from your progesterone injections and are not predictive of pregnancy.
      Please meet with your doctor and discuss the cycle details.

    • Depends on the next level. I am not sure how you got a level of 12 from a urinary test (which usually is qualitative and not quantitative). If a repeat hCG level shows an increase, you may have hope.

    • Hi Tanuja
      Congratulations! You are pregnant. However, it will take at least a couple of weeks to confirm that the pregnancy is progressing well. Your doctor will probably follow serial hCG levels and ultrasounds to confirm the viability, location and the number of gestational sacs present.

    Hi, I had my 2nd IVF, and this time the HCG level was 37.32 whereas my last HCG test showed 32.23 level from my 1st IVF cycle. When I met my doc he said that the chance again this time are very low and donot keep any hopes. Now 3 weeks are over after the embryo transfer i really donot now whether I am pregnant or not. Doc has just told me to visit him for ultrasound next week on Wednesday and that is the day when I will get confirmation about my pregnancy. Till then I have to wait….I dont feel any symptons of pregnancy but just scared if I bleed I might get a miscarriage. Please advise me for the same.

    • You should be getting a repeat hCG level a couple of days after the first. If the rise is normal. you may be okay. It is important to follow hCG levels till they are negative (< 5) as one needs to rule out a tubal pregnancy.

        It cannot be a tubal pregnancy since I have removed my tubes because they were damaged last year. I got my periods on 29th April’13, ET on 14th May’13 and HCG test on 29th May’13. its almost 1 month and 9 days i have not got my periods but I haven’t done the repeat HCG level test. Do u think I should do a repeat test or directly do my ultrasound on 12th June’13? and what are my chances??

    Hi there
    I’ve just undergone IVF, got 8 mature eggs, 7 fertilised, and 5 embryos. One embryo was transferred at day 2, which had 5 cells and no fragmentation. I had my first HCG test exactly 14 days after transfer, which was 16 days post fertilisation. My HCG was 40. I’m very worried that it’s abnormal or not going to last. I understand that it is the series of numbers that determine how the pregnancy is progressing rather than one number in isolation, except that I don’t know of people who have had a positive test that showed only 40 hcg. Have you known of anybody with levels like this that had normal healthy pregnancies?

    • Yes, we have seen patients with a first hCG level of 40 go on and deliver normally.
      It can be challenging to wait in your situation, but there is no other way.

    I had a day 6 transfer with 2 embryos. and my first blood test was 12 and my second was 48 is that normal will that effect the health of the baby?

    • You have not specified how many days after embryo transfer was your blood test done. I am presuming it went up from 12 to 48 in 2 days. This is a normal rise. A lot will depend on how you do over the next couple of weeks. We have had several patients with these numbers that have delivered normally.

    I did my first ivf cycle…and now i am day 12 after transfer of 5 days 3 embryos and my hcg level is 135 mIU/ml ..i told my doc about the result but he said he is not sure about that result coz in his lab the positive result is above ten and the lab i did the test in using different units …by the way the normal values the lab i tested my blood use : normal non pregnant hcg is 0.5 while 4 weeks is minimally 590 and mine is 135……now i am so afraid about my ivf success…am i pregnant or not

    I’m on 9th day after embryo transfer (2 embryos, 3 days old).
    I made my beta-hcg, the result is 17.
    I know its the minimum for this stage.
    It’s my first IVF after normal pregnancy 8 yrs ago…
    What do you thinki about this hcg level?

    • This question is similar to the one prior. My answer therefore is similar.
      You have to wait and see what the repeat test results are. The test should be repeated in 2 days and then a week later. If the rise is normal. you should have a positive outcome.
      We have had several patients with similar levels that have delivered babies.

        Heavy bleeding with clots (one soaked pad each hour) all day July 6th – July 7th…finally stopped July 8th
        Third hcg 1226 July 8
        Fourth hcg 2711 July 10
        Fifth hcg 1800 July 12 …dropped hcg
        What do you think is happening?
        I will repeat hcg on July 15th and ultrasound on July 17th

    Hi Doctor.
    After my first ivf attempt, I ended up having a ‘chemical pregnancy’ after an initial HCG level of 40, and three days later it had dropped to 9. On the day before the first blood test, I did not eat much as I was so nervous and terribly stressed. I ate very nutritious foods the rest of the time, but I am worried that not eating much that day and feeling so stressed before the blood test could have caused the embryo to fail and that’s why the numbers dropped and it ended. Could this have caused me to lose the baby?
    Also I have four frozen embryos, day 2. Do you think that the success chance is good since I actually GOT a pregnancy this time, rather than just a negative test?

    1st IVF chemical pregnancy, 2nd IVF failed.
    before 3rd IVF cycle hysthoscopy was performed polyp and fibroid cleared out.
    This is our 3rd IVF cycle, egg collection done on jun-10, 2 embryos got transferred on jun-14-2013 (Friday), today (jun 24) we did a HCG test result: 109 (10 days). Doctor suggested to go for 2nd HCG test on Jun-26 again. Also, Doc suggested to continue Crinone gel and folinal plus.
    Please let us know your suggestions and thoughts. We’re hoping for the best.

    • First of all — congratulations! With a first positive hCG level, you have overcome a huge hurdle. It is a good number. However, having experienced a chemical pregnancy, you do know that it is too early to celebrate.

        Today we had repeat of HCG test… and result is 346. Doc suggested no more HCG test is required and asked us to come on Jul-12 for scanning.

        • That is a good rise.
          At InVia, we get a repeat hCG level a week later along with an ultrasound.
          We then repeat another ultrasound a week later.

          Doing nothing but an ultrasound 2 weeks later is okay.
          More monitoring does not change the outcome, but it does detect problems sooner.

            We just went though our first IVF cycle. We had 6 healthy embryos and put two in on the 5th day. This was 10 days ago. So, we are 10 days after transfer and I had a blood test this morning. My HCG level is at 88. I am extremely nervous because I have had two miscarriages before with low HCG levels. The nurse said that anything above 25 was good, but didn’t give anything else to go on. I have to wait all weekend and go in on Monday.

            Is a 88 HCG level 10 days after a transfer (5 days, not 3) OK? Or would you like it to be a little bit higher. I just want to managed my expectations.

          • First of all, congratulations! With a positive pregnancy test you have already crossed a huge hurdle. 88 is a very common first hCG number 10 days after transfer. What is important is the follow up hCG level(s) and ultrasound(s).

            We have had hundreds of pregnancies that have started with even lower levels and have continued without problems.

            At the same time we have had several patients whose first hCG level was even higher that have miscarried.

            6week 1 day – CRL .49cm
            7week 1 day – CRL .98cm
            FHB +ve as per the doc remaining everything is fine. next scan is due on 7/31.

            Prescribed medicine:
            cyclogest (night 1)
            folinal plus (Daily 1)
            shellcal 500mg (daily 1)

    Hello, I am using a surrogate to have our baby. She was implanted on June 15th.with 2 frozen embryo’s. One 8 cell and one 7 cell on day three. she went today for her first blood test being 13 days after transfer, her test is positive, her HCG is at 350. is that a good number to start with?

    this was our first full ivf cycle and a day 3 transfer (3 embryos, 2 great 8cell , 1 eh)…on day 12 the hcg was 21 :O! but by day 15 (yesterday) it was at 64….. :)? is there anything to be happy about yet?

    next beta is tuesday… what should my symptoms be at this point…? (im on crinone and estradiol 2x/day)

    As noted below, our first test 10 days after transfer was an 88.6. 72 hours later my level was at 180… so just barely doubled after 72 hours. I would have liked to have seen a higher number that would have allowed me to take a deep breath… but I guess it’s still the waiting game until Wednesday morning for our 3rd test.

    I had a transfer on June 7 of this year. The egg retrieval was on June 2nd. I tested positive and today July 3rd they said I’m 6 weeks . How can that be when the transfer was done a month ago?

    • This is an excellent question. Gestational age, historically, has been calculated from the first day of the last menstrual period (and not from the day of conception). For IVF pregnancies, it means adding two weeks to the day of embryo transfer. So, the day you had your transfer, you were already considered to be 2 weeks pregnant!

    I’m hoping you can give me some direction. I am 31 years old. I had a miscarriage at 8 weeks in 2007, a successful and healthy pregnancy that resulted in my daughter (now 2 years old) in 2010, a year of unsuccessful attempts to get pregnant with a second child, a chemical pregnancy in May ’13 (following a salpingectomy of my left tube) and then a chemical pregnancy today following a successful IVF cycle (transfer of 2 high quality embryos). My fertility specialist is unresponsive as to what my next steps are and I would like to figure out if there is a reason why I have now had 3 miscarriages (two chemicals in a row). Do you think I should begin testing for chromosomal abnormalities? Would that be uncommon since I have a healthy daughter? Should I look into implantation issues? I had a C-section with my daughter but in a previous surgery, my OB noted that my ovaries / uterus look healthy. I am obviously heart-broken, but want to be aggressive in finding solutions to prevent repeat losses. Any feedback would be wonderful. Thanks!

    • I Am Sorry Your Recent IVF Cycle Resulted In A “Chemical” Pregnancy. I Am Presuming You Mean Your HCG Level Was Positive And Then After A Brief Rise, The Numbers Started Decreasing. Also, The Pregnancy Did Not Progress Till It Was Visualized On Ultrasound (When It Is Called A “Clinical” Miscarriage).

      It Is Unclear To Me Why You Are Describing The Pregnancy In May’13 As A Chemical Pregnancy. If You Had Your Left Tube Removed (Salpingectomy) It Was Probably An Ectopic (Tubal) Pregnancy.

      Based On What You Have Told Me, You Do Not Meet The Criteria For A Diagnosis Of Recurrent Miscarriages (Two Or Three Consecutive Miscarriages). The Chance Of You Having A Chromosomal Abnormality Are Slim At Best.

      To Be Evaluated For “Implantation Issues”, You Should Have A Hysterosonogram, Hysterosalpingogram Or A Hysteroscopy. Please Ask Your Physician Whether Your Endometrial Thickness On Day Of HCG Was At Least 6 Mm. Currently, Immunological Tests (NK-Cells Etc) Have Not Been Shown To Be Useful For Evaluating Implantation Failure.

      It Can Indeed Be Heartbreaking When An IVF Cycle Fails. You Are Young, Have Recently Had A Baby And Have Produced At Least Two High Quality Embryos. All These Are Good Prognostic Indicators. You May Need To Try IVF Again.

      Please Get A Second Opinion If Your Doctor Is Indeed “Unresponsive”. We Would Love To Have You As A Patient At InVia.

    • The number of embryos transferred is high compared to the routine at InVia (1 or 2).
      A hCG level of 78 is a reasonable starting point. What matters is the rate of rise of hCG levels over the next few weeks.

      There are no symptoms that can reliably differentiate between pregnant vs. not pregnant. Symptoms such as breasts feeling full, tiredness etc are usually due to the progesterone replacement medication.

    Good Morning

    So happy to have found this site. Any insight you could give be would be very much appreciated!
    So here’s my story.

    June 20 : transfer 3 embryos this is my first IVF

    July 2 : Hcg 102 the physician told me It was good and positive

    July 5 : HCG 182

    July 9 : HCG 620

    Right now my bubbies are sensitive and little bit bigger , don’t bleeding and sometimes cramping

    The Endocrinology will order me other HCG on July 13.

    I’m really concerned because He told me that HCG must be between 800 and 1000. They said I’m technically Pregnancy,

    I had made all the things and followed all the directions .

    Why my HCG don’t increase ? What could the reason be that it didn’t double? this a bad sign? Should I be concerned or do the beta levels seem okay?

    I’m so sad and worried about that

      The hCG values are low and the rise is abnormal.
      This indicates that it is most probably an abnormal pregnancy.

      If it is a chemical pregnancy, the numbers will start to decrease and then continue to fall till negative.

      If the numbers continue to rise, it is either an abnormal intrauterine pregnancy or a tubal pregnancy.

      My best advise to you is to please keep your follow up appointments till this is resolved.

I realize they haven’t doubled but I’m a bit confused on the conversion as the Internet websites are in US measure nuts. I live in Canada and on he metric system. Are my test results concerning as I’m going for my 3 rd blood test on Monday(15th) because of the slow rise?

    • The units used may appear different but they really are not different. IU/L is the same a mIU/mL.
      A rise from 125 to 146 IU/L in 2 days is indeed slow. It could be due to an abnormal pregnancy. However, it can also happen in a twin pregnancy where only one of the babies is continuing to grow.
      The third blood test level is critical.
      I do hope it shows an appropriate rise.
      In any case, please keep your follow up appointments with your doctor till this issue is resolved.

        • Welcome to the blog!
          You should have an ultrasound to confirm the presence of an intrauterine pregnancy.
          At this point we should be able to see at least a gestational sac.
          Your last test did not double but the other numbers all look good.
          It is possible you were pregnant with twins and one of the babies has stopped growing.

          If the pregnancy does not progress to a point where a gestational sac is seen on ultrasound, then it will be considered as a chemical pregnancy.

    I did my FET on 2 July (with the 2 eggs retrieved on 16 Jan and frozen on 18 Jan. Are they called day-2 eggs?). I did the HCG test this morning (16 July, 14 days after FET) with the result 140. Is the data relatively low? I saw many successful IVF patients got more than 200 or even 300… Is HCG the higher the better pls? Anything I may do to keep the embryo(s) grow healthier inside my body? The for your advice.

    • 140 is a good number.
      It needs to show an appropriate rise over the next couple of weeks after which ultrasound will be used to confirm that the pregnancy is progressing well.
      Please take all the medications prescribed by your doctor. A healthy lifestyle is always a good idea.

          Hi Dr,

          i found the the Crinone residue released was in red on 18 Jul nite (16 days after FET). After consulting Dr the next morning, i was given an injection on spot, more medicines and was asked to take bed rest. Some little brown or pink Crinone residue was found between 19-21 July. Since a little blood spot was found yesterday morning (22 July), i was asked to do 2nd hcg test. Luckily, no more colour was seen afterwards.

          My 1st HCG on 16 July 9am (14 days after FET): 140

          My 2nd HCG on 22 July 3:30pm (20 days after FET): 888

          Is the increase on track or a bit slow? If it is too slow, what is the possible outcomes? Can i do anything to improve the situation? Really worrying!

            Your first hCG level is good and the rise so far is within the appropriate range.
            You should be having an ultrasound shortly to confirm the pregnancy is in the uterus. One week later, you should have a more definite answer on the number and viability of the pregnancy.

I had my first ivf (2embies/blasto)transfer on Jul 8, cramping on the 15th, full period, clotting the whole 9 yards on the 17th. Stopped taking the progesterone supp. and did my first beta test today, (11days post transfer). Nurse called back with results and advised to continue taking progesterone supp. as results showed I was 15. She advised they need results to be under 5 to give me a negative. I’m 99.9%.sure I’m not pregnant, and now I have to go back in 2days to do another blood test. Is there any chance I can still be pregnant? I had come to terms I did not conceive, now this?!     • Congratulations! Your pregnancy test is indeed positive. You have successfully overcome a big initial hurdle. It, however, is a bit early to celebrate. Let us hope the levels rise normally and serial ultrasounds confirm it is a normal pregnancy. The number of embryos transferred was a bit high. We usually limit the number of embryos transferred to one or two.

      The symptoms you have while waiting for a pregnancy test are from the progesterone and estrogen replacement medications. There no diagnostic symptoms of pregnancy while waiting for a pregnancy test result.

    • A rise from 48 to 56 in three days is abnormal. To make a definite diagnosis, however, we will need a couple more blood levels.

    • Your doctor is worried about the rise, which is why you are being monitored so carefully.
      The last number is encouraging.
      What remains to be seen is the subsequent rise in hCG levels and serial ultrasounds.
      Please keep your monitoring appointments.
      It is only over the next couple of weeks that you will get a definite answer as to whether or not the pregnancy is progressing normally.
      I know it can be tough to wait.

I am schedule for a transfer tentatively for 8/3 but they may push to Monday 8/5. I know it’s too early to determine but with so many eggs fertilized is that good and what is the outcome using ICSI?

    • You are doing well in terms of number of eggs fertilized.
      With these high numbers, you may be at risk for ovarian hyperstimulation. Please check with your doctor as to what steps have been taken to avoid this potential complication.

        I tested positive on my pregnancy test on 8/14. On 8/5/13 2 ebroys were transferred 5dayblast at AB and AC. On day 14 my estrogen levels were 841, progesterone 8.6, beta 118. Based on the results my suppositories (pogesterone) were increased to twice in the am and pm. Today my levels estrogen levels are 1050, progesterone 17.8 and beta 171 Is this a viable pregnancy?

        • Yes it can be a viable pregnancy. Good luck with the follow up hCG levels and ultrasounds.

          FYI, serum progesterone levels can be low when progesterone vaginal suppositories are used. The progesterone gets absorbed directly by the uterus with very little entering the general circulation. Studies have shown that there is enough progesterone on board (in the uterine lining) even when the serum levels are low. We do NOT routinely measure progesterone levels in our IVF patients.

      Your hCG level is indeed low.
      I do not understand why you are being asked to take another hCG injection.
      It will only result in a false elevation of your hCG level.
      I have had pregnancies that have started this low progress normally.
      However, the most likely outcome is a chemical pregnancy.

    After 9 years of fighting with IF to try for a second child I went in for my first IVF in July 2013. I did a 5dt on 27th July. My HCG on 10dpt5dt (6th Aug) was 28 and 13dpt5dt (9th Aug) was 36 and 17dpt5dt (13 Aug) is 147. Can you please tell me if this is a viable pregnancy based on the HCG levels?       I am worried that you may be having what is a “chemical pregnancy”. Please keep your follow up appointments as a tubal pregnancy needs to be ruled out.

    I am 39 years old. I have 2 blocked F.T’s and Stage 3 Endo. On July 31 I only had 5 eggs retrieved. All fertilized. They transferred 3 embryos on day 3 (Aug 3rd) Today(12day post 3 day transfer) I got a call that I am pregnant. She told me level was 59.5. They did say they would like it to be about 100. My doc considers 3 and under to be negative so I will still retest on Monday. Although nurse said this is “not encouraging” there is still a bit of hope. Possible late implant, lab error etc could be possible. Have you seem successful pregnancies with such a low first beta?

    It was my first IVF-ICSI, here in Abu Dhabi. I have 5 healthy embrayos. On 19 Aug 2013 (Day3 after Egg Retrieval), They transfer 3 healthy Embrayos. After 5 days (24 Aug 2013) I took my Blood Test for my hcg level and on the same day (at night) my clinic advise me that my first Blood Test is Low Hormone. and suggested me to visit the clinic on 25 Aug for my hormone injection and to have additional medicine to take. They also advise me to take 4x a day for Estradiol (2mg) instead 3x a day only.

    Is there any chances to get positive result, please advise. Many Thanks.

    • So you had a retrieval on Aug 16, embryo transfer (ET) 3 days later on Aug 19 and a hCG level 6 days after ET on Aug 25. The hCG level on Aug 25 was “low” and you were then asked to take additional hormone injections (?) and the dose of estradiol was increased.

      I am not sure I have enough information to give you a specific answer.

      Yes, your test result can come back positive.

      Please keep your follow up appointments and I hope the levels will rise normally.

    • Yes they do.
      I do not understand why they checked your serum progesterone levels when you are on Crinone. Crinone gets absorbed from the vaginal wall and gets preferentially delivered to the uterus. Very little Crinone actually makes it to the general blood circulation.

    • We will generally do a first hCG level 14 days after retrieval (approximately 9 days after blastocyst transfer). We repeat a hCG level two days later and a first ultrasound a week after that.
      In your case, (I am presuming it was a blastocyst transfer) the first hCG level would be on Sept 3, follow up hCG on Sept 5 and first ultrasound on Sept 12. An second ultrasound done a week later will be confirmatory.

    I am 41 and on 26 Aug 2013 my nurse called me to tell my pregnancy test was negative
    I had a 5 day 1 embryo blast transfer. I was having symptoms till the 8 day and on day 8 when I was making dinner I realized 2 times sharp pain ,last very few seconds, in my uterus and the next day morning all my symptoms were gone. I was all normal. The nurse said its normal but I m thinking that why the symptoms went all of a sudden and not gradually if it was the meds symptoms
    Day 13 was the worst day when I saw bright red blood clotting or discharge. I called the nurse and she said probably my periods are starting but asked me to continue the estradiol and the progesterone inserts.
    The next day any way was a hell day to listen the pg test was -ve
    We had 2 embryos to transfer but we decided to transfer only 1. But the other embryo also did not continue to grow and we have no frozen embryo now.
    After the embryo transfer after 24 hours were over I had to climb stairs in my house at least 6-7 times a day as we have 2 levels in my house. Do you think that was an issue. I also had to cook food daily because my in law is here and we haven’t told anything to her. Could you please guide me shall I undergo another cycle and if yes what precautions shall I take. I don’t eat junk or fried or greasy. I drink tea 2 times a day but with milk ( Indian black tea) shall I stop it?
    Shall we go for icsi as my husbands sperm count was low but the second time test it was not low. We did not do icsi because we were scared to hear the side effects and chances of defective baby. Please doctor advice me what shall I do?

      I am sorry your IVF cycle was not successful. You are 41 years young and it is not unusual (in your age group) to require more than one cycle to conceive. Good luck for your next IVF attempt!

      Your symptoms are typical for someone who is waiting for a pregnancy test after IVF. The interesting fact is that symptoms cannot differentiate between a positive and negative outcome. It is therefore important for patients to continue their medications and keep the first pregnancy test appointments.

      Your outcome was NOT affected by any of the activities you have mentioned. So please do not blame yourself!

      Drinking two cups of tea with milk is okay.

      Please discuss ICSI with your fertility specialist, I do not have the information to give you specific advice.

    • At this stage of pregnancy, the ultrasound is more predictive than the hCG level.
      As long as the baby is growing and there is a heartbeat, you should do well.

    We recently transferred 2 healthy blastocysts into a carrier surrogate late in August 2013. After 9 days post embryo transfer a beta blood test was performed and the beta value was 9.8. We are getting a second test in 2 days to find out if it has changed. What would your opinion be of this beta value post 9 days after embryo transfer?     • When we have patients on IM progesterone, we like the progesterone to be greater than 15 ng/ml. With vaginal progesterone, we do not measure progesterone levels as the medication is absorbed directly to the uterus.
      The hCG level is the one that is now important.

    • The numbers are not good.
      Your nurse is correct. If both embryos implanted and only one is progressing, there can be a low rise followed by appropriate numbers.
      This will more likely be a chemical pregnancy, but you can never be sure.

    i found out on the 12th of this month that I am pregnant. By my calculations I am about 5 weeks along (could be less though), however last night (the 14th) i began bleeding and having mild cramps. The bleeding is not very heavy, but not light either. I went to the ER and they took my HCG level and it came in at only 9. My DR did not check my cervix, to see if it was opened or closed and did not suggest another follow up test and told me it was definitely a miscarriage or ectopic. I found him to be really cold about the matter, and am seeking further opinions and a follow up blood test and possibly an U/S. As I have read many different articles, some encouraging and some not so. Am i definitely going to loose this baby?

    • At 4 weeks from your last menstrual period (i.e. when you miss a period), the hCG level should be close to 100 mIU/mL. At 5 weeks, your hCG level (9) is very low. Also, with the bleeding, it does look like you are miscarrying or having an ectopic (unlikely). I am sorry your doctor seemed to be cold. You should get a follow up level and be monitored till the pregnancy test is negative. Please get a second opinion.

      • If your pregnancy is uncomplicated, it is okay to travel at 18 weeks gestation. Please make sure you get up and walk periodically to minimize chance of blood clots. There are many flight attendants who are pregnant and flying regularly during the second trimester.

    • Please have a specialist examine you and do an ultrasound. If an intrauterine pregnancy is seen, you may be okay. As you are well aware, a gestational trophoblastic neoplasm needs to be ruled out.

      The test two days from now may give you a better idea.

Today, 72 hours later, I had my u/s which showed intrauterine pregnancy and the hcg was 183. My Dr told me it’s not a healthy pregnancy and that I need to stop the progesterone and estrace. He will continue to monitor hcg till they reach zero. Is there anyway that this could turn out to be a normal pregnancy?

          • It could still be an ectopic. The “pregnancy” seen in the uterus could be a “pseudosac” — thickened uterine lining that appears like a pregnancy, but there is no fetal pole. Please keep your follow up appointments.

    • Congratulations on your positive pregnancy test! It is indeed a great number. However, its too premature to celebrate as the follow up visits need to show appropriate progress. Mild cramping is ot unusual and it is usually teh enlarged ovaries. It is a bit unusual that your physician scheduled a follow up 3 weeks later. Why don’t you let him or her know about your cramping and request an earlier blood test?

I also do not understand why you have been given hCG booster shots — these can falsely elevate your hCG levels.     • Please contact your doctor and ask for an earlier appointment. You need a repeat hCG level to confirm that everything is okay.
      Slight bleeding is common even in normal pregnancies. It does not necessarily mean the pregnancy is not doing well.

        • The rise is from 96 to 373 in 3 days. That is an appropriate rise. I would be cautiously optimistic in this situation. Please continue your medications and get a repeat hCG level in a week, this time with an ultrasound. A gestational sac should be visualized with the hCG level being much higher. If the bleeding gets worse, please request an earlier repeat hCG level.

    • These hCG levels unfortunately are low.
      You are probably having a chemical pregnancy. A tubal pregnancy can also present this way.

        • I would NOT use Trenaxa for the bleeding. Trenaxa is indicated when there is dysfunctional uterine bleeding (no cause detected) and NOT for pregnancy related bleeding.
          Your first hCG level is okay. It is not unusual to have bleeding at this time. Things are usually okay if the hCG level shows appropriate rise over the next few days, weeks.

          • That is not a good rise.
            You need to keep your follow up monitoring appointments till the situation is resolved.
            This is probably a chemical pregnancy although an ectopic is also possible.
            My hope is that the subsequent rise is normal and the pregnancy progresses normally. This can happen in a twin pregnancy where one of the pregnancies does not develop (hence the abnormal rise in hCG level) and the other does.

    • I am sorry your first IVF cycle ended as a chemical pregnancy. Your first (?) hCG level (373) was actually very good.
      I am glad you have cryopreserved embryos. This gives you another shot at pregnancy without going through another stimulation and retrieval.
      The answer to your question will be different for each IVF Center and vary based on the patients age and the quality of the cryopreserved embryos.
      At InVia, our frozen embryo transfer rates vary from 25 – 50%. Please ask your physician what their success rates are.

    I’ve just completed my 3 rd IVF cycle, it turns out I’m pregnant but my HCG started out low, first beta on the day of pregnancy test was 49, 48 hours later 85 and the third was at 266, so it got a little better but I’m due for an ultrasound (my first) in a few days and I’m VERY nervous, only because I don’t at all feel pregnant, I have NO symptoms!!
    I really don’t know what to expect as the HCG started out so low. I had 2 blastocysts transferred on day 5 ..

    • It can indeed be a trying time because you so badly want the pregnancy to progress normally.
      What I can tell you with certainty is that not having any symptoms is not a problem. We have had plenty of patients that had no symptoms and have gone and delivered normal healthy babies.

    but my lower abdominal pain mainly rt side and vaginal region still now.he neglected and advised to take Drotin 3 times daily. but there is no effect.

    Im so glad to have found this blog but am not very hopeful for the reply as i cant mention my email iv a son whose a teenager and was naturally conceived. Then i had three ectopics and ten miscarriages. Chromsomal analysis gud. Im 35 yrs old. Now my 14dpt is negative but am not having periods although im always on time. Three embies were transfered. Besides im having low grade fever. Cant have ectopic obvsly. Can i hope for that late implantation. Im four days over due and have viginal pain and 99 fever which is persistant. Can u please reply. Many thanks in advance.

    • I do hope you have had a complete work up for your miscarriages! IVF is definitely a good option for you after having had 3 ectopics.
      It is not unusual for a period to be delayed for a few days especially since you were probably taking progesterone supplement. Your period can be heavy with clots and cramping.
      A temperature of 99 is normal (a fever is diagnosed when temperature is above 100.4).
      I am sorry your pregnancy test was negative.
      It is not unusual to require more than one IVF cycle to achieve success.

    • I cannot give you a definite answer as the hCG support shots are probably contributing to your hCG level. We do not give hCG support shots at InVia — they increase the risk of ovarian hyperstimulation syndrome. A better option is to give progesterone (vaginally or by IM injection). I therefore do not have personal experience with how much residual hCG is left during support shots.

      Your blood level is similar to the one you had with your son. I would therefore suggest that you follow your doctors instructions and hope for the best.

        I am using a surrogate to have our baby. She was implanted on Sept 28, evening with 2 frozen embryo’s. One 8 cell and one 9 cell on day three.

        she went on 11 Oct(morning) for her first blood test being, technically, 13.5 days after transfer, the results are:

        PRGE:18.1
        HCG:7.9 (reference 0-3)
        I understand this might to be a good starting results. However it is beyond what the reference range. I read most of your comments as this is not positive. My question is that why the first HCG test’s reference is given 5mIU/mL anyway. Even around 25, your interpretation is skew to negative. and such reference really give me misunderstanding and unnecessary confidence.

          It is a very low positive. At this point, the best option is to keep the follow up appointments and hope for the best.             76 is a “mean” level reported by the researchers. The standard deviation is 54! There is also a wide range from (5 – 863). This indicates that 76 is not an absolute level to differentiate between a viable vs. non-viable pregnancy. It is only a “guideline” value. Patients with lower first hCG values do go on to have normal babies. Hope this further clarifies the issue for you.

            As I mentioned earlier, on 14 days (11 Oct) after transfer, the results are:

            My surrogate went to blood test today, 17 Oct ( 6 days after 11 Oct), or 20 days after transfer, the reading changed, hCG reading goes up.
            PRGE:14.6 (drop from 18.1)
            HCG:355.5 (increase from 7.9 in about 5.5 days)(reference 0-3)

            It seems a bit promising, what we can do is still wait and see. Any comment from you?

            However, 355 is still a bit low for 20 days after transfer.

            At 4 weeks after a transfer, ultrasound will give a fairly definitive answer.
            At that time a gestational sac should be visualized.
            Quite often a fetal pole with fetal heartbeat is also seen.
            hCG values are less predictive.

            My surrogate mother went for a ultrasound check today and there is no gestational sac visualized. Unfortunately, she hasn’t do a blood test, so don’t know hcg reading. The Doc seems to indicate that the early doubled hcg was due to the hcg assistant injection. And that doc suggests no hope for this case.
            Do you think is there still a hope?

It was my first ivf….

    • I understand what you are saying. “I did everything I was asked to. Why am I not pregnant?”. The fact of the matter is that even in the best of hands, IVF does not have a 100% success rate. Sometimes it takes more than one cycle to achieve pregnancy. Please meet with your doctor and have a discussion about the cycle. Can the stimulation be improved? Was there a problem with egg quality? sperm? embryo cleavage? Why did you not have embryos to cryo-preserve? If you do not get satisfactory answers, please get a second opinion. We would welcome you as a patient at InVia.

          • I am sorry your test result was negative.
            Have you stopped taking your progesterone? If you continue taking the progesterone (injection or vaginal); that can postpone getting a period.
            Please make a follow up appointment with your physician to discuss the cycle. If there is a problem with egg, sperm or embryo; corrective measures should be discussed and an appropriate plan made for the next attempt.

    • We have had hundreds of patients with endometriosis conceive and deliver healthy babies. Severe endometriosis (especially with ovarian cyst (endometrioma)) has a lower pregnancy rate especially if the egg quality is affected.
      Please meet with your doctor and find out why only 4 eggs were retrieved.

    • That is not an appropriate rise in the hCG level. You will get a more definite answer in the next couple of weeks.

    Am 43 years old, currently in my 3rd IVF trial. We had 3 embryos (4 days old) transferred on 4 October 2013. Apparently of very good quality. I did first blood test on Wednesday 16 October and my Beta levels were 330. According to my clinic this level is a good positive. However since this morning I feel less symptoms so am a bit concerned. Usually I tend to feel the pull in the ligament of my belly and more bloated. But Since this morning my belly is more relaxed. Also so far I wasn’t sleeping well but yesterday I had a good nite rest. My next blood test is on Monday 21 October. Also a bit concerned that I have no other pregnancy symptoms like nausea etc….hope the embryo is still growing healthily. Thanks for your guidance

        Hello again Dr, i had my second blood test today (21/10/13) and it was 856. According to my doctor the beta is not increasing sufficiently. To recall, i had:
        Transfer (4days old embryod): on 4 october
        1st blood test: (16/10/13) at 322.
        Today however the progesterone level was greater than 60 & oestrogen > 2000.
        I have blood test again on wednesday 23/10
        Thanks for your advice as I feel am loosing some of the few symptoms I was having & am getting worried

These days I’m getting anxiety filled dreams and last night I even dreamt that I had menstruated. Does it mean that I am not pregnant? My previous IVF cyle had failed (beta hcg was 39 initially and it reduced to almost half (19) after 48 hours.) I had conceived last year through IUI only to suffer from a miscarriage after 17 weeks. After that 2 IUI’s and 1 IVF failed. I am already 38 and feeling terribly anxious.

      You got very close! Its a question of whether the cup is half empty or half full.
      Your options are to try again or see if you are a suitable candidate for a tubal reversal.

My wife and I just went through our 3rd IVF cycle.. we just had our first blood test on Monday Oct 14th and our levels were at 350… they did not schedule us to come back until the following Monday.. I take it that this is excellent news since we did not have to come back every couple of days for monitoring… my question is, since we don’t come back for a week, what should be the acceptable number for us upon our next test? (we had two eggs transferred quality was 6-8)?

    • Congratulations, and welcome to the blog! 350 is an excellent first hCG level. The level should double every 3 days or so. There however, is a wide range and every pregnancy is different. In two weeks from now, vaginal ultrasound should show a gestational sac(s?. There should be a heartbeat visualized a week later.
      Please keep your follow up appointment and continue your medications as instructed by your doctor.

    I had my first blood test on October 16, which was 13 days post 3 day transfer of two compacting embryos, and it was positive at 334. On October 18, blood test was only 337. I was told it could have been twins, but now is only one. What are the chances of that? In your experience, is it more likely a chemical. I go for a 3rd test on October 21.

    • Congratulations! The first hCG level is excellent (337) but the second is not.
      You have been given correct advice.
      The third test level is critical, it should double every 3 days or so.
      I wish I could give you a more specific answer for you.
      We have had patients with levels like these that have had normal pregnancies.
      Please do not give up hope, keep your follow up appointments and follow your doctors instructions.

    Dear Doctor, A day 3 embryo was transferred 12 days ago. My HCG level is 7.5. We’ll repeat the test in two days. Even though this does not turn into a healthy pregnancy can I still be happy because it means that implantation actually occurred? This is my 3rd IVF and all the previous ones were zero.

    • If your next hCG level is higher than 7.5, then implantation did take place.
      If the next level is lower, then this is probably residual hCG from your trigger shot.

          This could be a chemical pregnancy or a tubal pregnancy so it is important to keep your follow up appointments.

    I am scheduled for my 2nd hcg and first ultrasound on Nov. 1st. My doctor says by then he’s expecting to see my levels at about 10000. Do you think thats too far of a wait between my 1st and 2nd test? Is 1652 too high?

      Congratulations! It is indeed rare for a 45 yo to conceive with her own eggs. The pregnancy rate is about 1%!
      You are obviously in very good hands.
      The first hCG level is a bit high but nothing to worry about. It is not unreasonable for the level to be much higher a week later.
      Your best bet is to follow your doctors instructions, take your medicines as advised and keep your fingers crossed.

    • See if you are a suitable candidate for a tubal reversal.
      You would need a HSG + the dictated operation note from your C-section which would describe how big a segment of your tubes was tied.

    I had Oocytes retrieval on 17-Oct-2013 and six(6) embryos transferred on 19-Oct-2013; after 14 days of transfer (2-Nov-2013) Beta HCG levels are at 54.60, Nurse said it should be above 100. We had ICSI with Laser Assisted Hatching for embryos.
    I am very much disappointed to hear this, is this bad sign for pregnancy?

      Congratulations on having a positive first hCG level!
      It, however, is too early to celebrate but you have successfully overcome a huge hurdle.
      Basically, what the nurse is saying is what is specified in the blog. The “mean” hCG level should be close to 100. There, however, is a wide “range” and we have had many patients with first hCG levels even lower than yours that have gone and delivered healthy babies.

    We transferred 2 perfect embryos on day 3 which was Oct 20th. Nov 4th we were notified that we were pregnant with a hcg of 1,000. Is this a normal number? My nurse was excited. I go back on 11/6 for another hcg and after that they will determine if 1 more test is needed or if I will just return when I am 6 weeks. Based on my first hcg count they determined I was 4 weeks, 4 days. Any concerns??

    I had A FET transfer ( 5 day blastocyst and we did PDG testing on our embryos) on Oct 25th and on Nov 4th my HCG was 17.1 . I was advised to come back today Nov 6th and the number was 31.2. They have now told me to come back for a third test on Friday. Any reason to be hopeful? They are not optimistic and have mentioned ectopic pregnancy. This is my first IVF cycle.     • The numbers are all low.
      It is reasonable to be cautiously optimistic — hope for the best but prepare for the worst.
      At this point your best bet is to continue taking the medications and keep your follow up appointments.

      Note: I am currently taking 200mg of progesterone (tablet) 1 mL of pregesterone (injection) Bottle reads 50 mg/mL and Delestrogen every monday and thursday .4 mL (injection) bottole reads 20mg/mL. and that is it nothing else…. thank you soo much for your time cant wait to hear from you :)

    I am 39 and my husband is 34. I have a 10 yr old daughter from a previous marriage and my husband has no children genetically from him. I have had 3 laparoscopies and do have endometriosis (managed). We have been trying to conceive for three years. 3 unsuccessful IUI’s and then we moved on to IVF. 11 eggs were harvested, 9 fertilized, 5 made it to 5 day blast’. We transferred two in August, unsuccessfully. We transferred the remaining three frozen’s October 19th. First beta was 25 (11 day post transfer), second beta was 51 (13 days FEtT), third was 193 (17 days post FET), and the 4th was 276 (19 days post FET). My RE says he is 100% sure I am miscarrying and wants me to withdrawal support, but I want to continue to day 23 hoping we can see with a sonogram. Would anyone agree with this? I still have all of the same symptoms I have had…tender breasts, wake up thirsty and a bit hungry, tired, etc… please advise! Anyone else in a similar situation?

    • The hCG rise has not been appropriate, especially from days 17 – 19 post FET..
      A theoretical possibility is that the lab could have made an error in the reporting.
      It is therefore reasonable to give you the benefit of the doubt and have you continue your meds for a few more days.

    • Congratulations! Your levels are normal. This is a good sign. It is okay to repeat the level a week later. At InVia, we measure hCG levels 2 days after the first and if the rise is appropriate, check the level a week later.

    Im in my second cycle of ivf, first one was chemical pregnancy. On the 10th day of my period, the endometrium was 8.3, but on the 15th day it is dropped to 6.3, Doctor, please let me know as to why there will be a drop like this and are there any complications because of this.

    • Uterine contractions can change the endometrial thickness measurement. Also, there is an inter-observer difference — if the same scan is done by two different ultrasonographers, there will be a slight difference in the measurement.
      Remember we are talking about “millimeters”.
      I do not see any cause for concern with this observation.

      Most IVF clinics recommend continuing progesterone supplementation till 9 – 11 weeks of pregnancy. However, there are some (rare) physicians that will stop the progesterone much earlier.
      Your first hCG level was excellent. It is possible your bleeding is related to your stopping the progesterone. Please speak to your doctor and see if he feels you should restart your progesterone supplementation.

    On day 14 my HCG level came 11 and my clinic asked me to test again on day 18. The HCG dropped to 4 and they said it was chemical pregnancY. Was HCG 11 even a good level?
    Unfortunately out of the 10 froxen embryos my RE said only 2 look good and we are thinking about doing a FET next. However, am very uncertain and skeptical that it will work this time. I’m trying hard to be positive.

    • We like the hCG level to increase before it drops before we call it a chemical pregnancy e.g. from 11 to 18 before it drops to 4. Otherwise it could just be the residual hCG from your trigger shot.

      Please go ahead with your FET. Things are not always logical — even “not good looking” embryos can result in beautiful babies!

      I am surprised they retrieved only 4 eggs from a patient with PCOD and a h/o OHSS

    • I am sorry but the numbers are a bit low.
      You will get a more definite answer in the next week or two, but right now it appears that you may be having a chemical pregnancy (need to rule out a tubal pregnancy).

My doctor does not do them. He always does frozen cycles. 2 months later, Frozen Transfer 11/14 of 2 6 day ICSI embryos. Pregnancy test was 11/25, 11 days later. Came back positive with a beta of 96. So excited, first time ever being pregnant at 35 years old (hubby 28 no issues). Beta test 2 days later 11/27 13 days later is 106. I think they wanted it to double. They want to test again on Monday to see what 3rd beta says, Did only one keep growing. Is that possible? It didn’t go down, I was under work stress. I should have taken off the whole 2-3 weeks and not just 1. Work pressure the day of second beta, can this cause anything? They say i am still pregnant and to continue on with meds. also i hear that boys have lower HCG levels or this can happen too if 2 implanted at once and they other one is the one that kept growing. Any best guess advice would be great. I am holding on the hope. I’m elated i even got pregnant. I thought only positive but now that beta is not doubling. I’m still thinking positive but secretly worried. Thank you.

    • It is possible that (only one of the embryos is keeping on growing) is the reason for the slow rise in your hCG level.
      The only way to find out is to keep your follow up appointments.
      Please rest assured that it is very unlikely that you did anything wrong. The process itself can be stressful and taking 2 – 3 weeks off does NOT make a difference in the outcome.

        • I am sorry your hCG levels did not rise appropriately.
          I do understand why you doctor stopped your meds.
          However, at InVia, we would have brought you in for weekly hCG levels and followed them to negative.

    Also, My issue is blocked tubes. Both blocked and one was opened with HSG but not sure on the quality of the open tube, 3 IUI’s later nothing so i went on to IVF. This was my first IVF cycle. I might be a slow starter too the same lady called me and said i only made 6 after 7 days of stemming eggs. I kept going and they kept finding more each ultrasound. and he retrieved 18 in the end.           The hCG booster injection is sometimes given to boost the corpus luteum’s production of progesterone.
          The same effect can be achieved by giving progesterone (IM or vaginally).
          We do not use booster hCG injections as they increase the incidence of ovarian hyperstimulation syndrome and interfere with the pregnancy test result (false positive).
          Your best option is to wait ten days before repeating a pregnancy test.

    I have gone through IVF cycle, they transferred 2 blastocysts on 7th of this month and on 19th gone for blood test and results were 56 HCG levels, my nurse told it is very low and need to go for another test in 2 days time, after 2 days the results dropped to 20.7 and we were so broke down as we havent expected, dr told u were pregnant but it was miscarried, we are in shock. I asked my dr whether there will be any rise again, he said it may not be but go for another test in 2 days. After 2 days i again went for the blood test and results raised to 51 HCG level. We were so confused and dr didnt say anything and finally we decided to continue the supporting medicines(Utrogestrone)and my blood test will be again in week time ie it will be in coming monday. I am so worried what will be my results…..from 2 days i am cramping little to medium levelat abdomen and legs. Dr can you please tell me is it a healthy pregnancy? Is this kind of cramping(today leg cramping) normal or not….I am so tensed. Thanks in advance.

    • The hCG levels are consistent with an abnormal pregnancy.
      It could be an intrauterine pregnancy that is not doing well or a tubal pregnancy.
      Please keep your follow up appointments and follow your doctors instructions.

    • The nurses are appropriately concerned with your low rising levels.
      Three weeks after ET, ultrasound should show a gestational sac in the uterus.
      Otherwise, one must suspect an abnormal IUP or a tubal pregnancy.

    Hello, I’m 44 years old and my 5 day DE FET was on 12/18/2013. We transferred one early blast and a morula. My pregnancy test was on 12/28, which was 10dpt-my 1st beta #’s were 190, two days later they were 555. four days later they were 3,188. I know my RE said Congrats but do the numbers seem to be rising well? This cannot be considered “chemical” can it? I have had no bleeding or spotting with the transfer or after. is this a good sign? I’m scheduled for a ultrasound later this week but a bit worried as I have no real symptoms. I’m doing progesterone injections and Estradiol pills 3x per day and have never been pregnant before..If one or both didn’t “stick” would the numbers be where they are or would I have some bleeding?

      These numbers are terrific.
      There should be a gestational sac visualized in a week with heartbeat a week later.
      Your age puts you at a high-risk for miscarriage. However, so far so good.

We had detailed counseling with DR they said as estrogen accepted go beyond 2000 my mine was 890 so egg qty might effected to turn into embryo and sustain the growth so they said any way will try with freezes once next cycle. And also if that also won’t work we need to go for egg donor. We are not interested in egg donor my age 27 ,will it get better my egg qty naturally. We planning to stop all this and try naturally for one year and then take decision.

    • That is a very low number and it is probably an abnormal pregnancy.
      You are probably having a chemical pregnancy although a tubal pregnancy cannot be ruled out.

      Do you have an incompetent cervix. If so, please discuss cervical cerclage with your doctor and consider an elective single embryo transfer.

        I am using Surrogate , Egg transfer was done on 18th Jan 2014 and B-HCG on Feb 1st = 1800 and Feb 3rd as 4300 . The number looks alarming.. Is it twins ? triplets ? quadrapuls ? Please advice

        • Congratulations! Your surrogate is very pregnant.
          I do hope you did not transfer more than two embryos.
          The hCG levels are indeed high and could be due to multiples.
          However, I have had singletons with numbers like this.
          You will have to wait a couple of weeks for ultrasound to give a definite answer.

I had FET from donor eggs on the 20th of January and on the 5th of February I went for my blood test.My HCG readings were 4000 which scare as it looks so high, then again n the 8th on the day I was suppose to go for blood test I woke up and discovered that I was starting to bleed there was some clots and I experiences some cramps as well, I went to get tested doc told me tht my hcg was 11000. He then put me on cyklokapron and he advised me to stop taking ecotrin and booked me off for week at work for strict bed rest. Should I be worried is my hcg normal.

    Good mor Dr I was diagnosed to have SLE with pulmonary hypertension since 2006.i am under treatment .we went thro ivf cycle with minimal stimulation.dr said left ovary is solid ,right ovary is ok .they started stimulation. 2oocytes retrived .embryos of 5 cell staged & 8 cell staged were transferred to the surrogate.11th day hcg showed 24.dr told it as low positive.after 2days it came to 7 .pregnancy negative. shall i go for the next cycle

    I am 27 and Husbnad :30,facing male fertility problem(low sperm : 5mill).We did ICSI on 25 Dec 2013 ,got 5 grade A blasto,transferred 2 and freezed 3.But Bhcg came(10.61,27,4 and 12.5).

    We are thinking to do FET,May i know what should be gap in between IVF and FET.

    IS there any thing which can increase chance of success (Food or exercise).

    Whether Luprin injesction protocol is good thn new protocol?

    • I am sorry your recent IVF cycle resulted in a chemical pregnancy.
      You can start a FET cycle in the next cycle without further delay.
      It is okay to use Lupron.

        15th Feb – 1409
        17th Feb – 1932

        i went for ultra sound on 19th. Doc told me gestational and yolk sac can be seen in uterus. But as my HCG levels are not doubling as expected (every 48 hrs), she said we need to keep an eye and keep monitoring BHCG levels.
        Please advise how it can go and what are the chances of success.

        • I agree with your doctor.
          The hCG rise from 15th Feb to 17th Feb was abnormal.
          It is a good sign that a yolk sac was visualized on the 19th.
          By next week there should be a fetal pole present (sometimes with heartbeat).
          Since multiple embryos were transferred, there is always the possibility that more than one had initially implanted. However, only one may be progressing. Hence the (? temporary) abnormal rise in hCG level.
          Regardless, you should keep your follow up monitoring appointments, they need to make sure you do not have a concurrent tubal pregnancy.
          It is still possible that the pregnancy is normal. However, you will have to wait for a week or two.

    This is regarding my sister who went through first cycle of IVF with the following sequence of dates. Her age now is 36.

    19 May – Transfer of embryo
    03 June – 1st HCG test: It showed the level as 277.
    Doctor confirmed pregnancy.
    10 June – 2nd HCG test : It reduced drastically to 4 which was unbelievable and doctor confirmed negative and says the embryo did not develop well.

    Need your advice. Should medication be stopped?
    Is retest required?
    What should be the next steps, precautions and duration for the second cycle?

    • It looks like your sister had a chemical pregnancy.
      This is where the blood test is positive but the pregnancy does not progress to a point where it can be seen on ultrasound.
      This unfortunately does occur quite often after an IVF cycle.
      Your sister should reconsult with her doctor and discuss the next step.
      It is okay to stop the progesterone and there is no need for a retest unless you suspect lab error. Please have her follow her doctors advice on this one.

        Just completed my 2nd IVF cycle. 1st was normal IVF, 2nd ICSI. I had 1 AA grade, 5 day blastocyst transferred on May 24th and a confirmed pregnancy on June 4th with hcg of 132. 2nd hcg on the 10th was 787 and 3rd hcg on the 13th of 929. Hcg only increased by 18% in 72 hrs, is there any hope? Prior to IVF I had a natural pregnancy which resulted in a blighted ovum. We have unexplained infertility and whilst I am 42, I produce good quality eggs. If embryo is of high quality and sticks, why would the pregnancy not be viable? trying not to think the worst!

        • I am sorry this pregnancy is not progressing well.
          The commonest reason for pregnancy loss in your age group is that the embryos are aneuploid (abnormal chromosomes).
          You may want to consider doing pre-implantation genetic testing next time around especially if the other tests for recurrent pregnancy loss are all normal.

    Hi Doctor, I am pregnant via IVF ICSI. Exactly 6 weeks and I day today. My last HCG test this morning was right on the boarder at 1639 with progesterone level 383 (due to presseries). I went for an early ultrasound today to rule out ectopic and the technician could only see a 9mm sac in my uterus, but it was empty. Any chance it is too early to see anything or should I face reality? Thanks in advance for your feedback.

    • You have not given me the time interval between the blood tests.
      I am presuming you are worried that the hCG levels are not rising appropriately.
      They are indeed low.
      You may be having a chemical pregnancy, however, a tubal pregnancy needs to be ruled out.

        I did a IVF with 2 5A grade embryo transfer on 16th June. I did a Beta hcg test on Monday June 30th and my hcg was 29. Doctor said its less but I am definitely pregnant. I need to repeat the test again on wednesday to see if the hcg reading doubles. I got my results on wednesday and it was 24. So I asked the doctor to repeat the test again on monday July 7th and it was 89. Do you think its a case of vanishting twin syndrome? Why does the hcg drop and rise again? Also note that I had bleeding from 6/12/2014 to 7/1/2014. Please do reply.

    I had FET done on 16th June 0n 30th june my HCG WAS 178.4 post that i was advised to take sifasi 5000 shots every 4th day .on 4th HCG was 1136 on 8th HCG WAS 3092 AND ON 12 th it was 5103 is it good or bad

    • I am presuming “sifasi” is hCG.
      I do not understand why you were given these injections.
      At InVia, we supplement FET’s with progesterone and estradiol only.
      The hCG from your injections makes it difficult to comment on the hCG rise from the pregnancy.
      The numbers, however, are increasing and that is better than them dropping.
      An ultrasound in the next week or so will be needed to confirm things are progressing well.
